1. Our duties
GAB Medical Group is required by law to maintain the privacy of your Protected Health Information (PHI), provide this notice, and abide by the terms currently in effect.
2. How we may use and disclose your PHI
- Treatment: to provide medical care and coordinate with pharmacies and other providers.
- Payment: to process billing and insurance, when applicable.
- Healthcare operations: quality assurance, training and licensing reviews.
- As required by law: public health reporting, legal process, law enforcement.
- With your written authorization for any other use, which you may revoke at any time.
3. Your rights regarding PHI
- Request access to and a copy of your medical records.
- Request corrections to records you believe are inaccurate.
- Request restrictions on certain uses and disclosures.
- Request confidential communications by alternative means or location.
- Receive an accounting of disclosures we have made.
- Receive a paper copy of this notice on request.
4. Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at info@gabmedicalgroup.com or with the US Department of Health and Human Services. We will not retaliate against you for filing a complaint.
5. Effective date and changes
This notice is effective as of the date above. We reserve the right to change it; updated notices will be posted on this website and apply to all PHI we maintain.
