Telehealth Informed Consent

Please read this document carefully before scheduling your visit. By scheduling and attending a GAB Medical Group telehealth visit, you confirm that you have read, understand, and voluntarily agree to the terms below.
1. Nature of telehealth
Telehealth is the delivery of healthcare using interactive audio, video and data communications. At GAB Medical Group this includes secure video visits, phone calls, asynchronous messaging, electronic intake questionnaires, photo uploads, electronic prescribing (e-prescribing) and the secure transmission of medical records, lab results and imaging between you, your clinician and our partner pharmacy and laboratories. No in-person physical examination is performed during a telehealth visit.
2. Eligibility
- You are 18 years of age or older.
- You are physically located, at the time of the visit, in a U.S. state where the treating clinician is actively licensed.
- You have the legal capacity to consent to your own medical care.
- You will provide accurate, current and complete information about your identity, medical history, medications, allergies and current symptoms.
3. Benefits of telehealth
- Convenient access to a licensed clinician from home or work.
- Faster initiation of evaluation and, when appropriate, treatment.
- Reduced travel, time off work and exposure to other illnesses.
- Continuity of care through secure messaging and scheduled follow-up visits.
4. Risks and limitations
- Some conditions cannot be fully evaluated without a hands-on physical exam, in-person testing or imaging, and the clinician may need to refer you to in-person care.
- The clinician relies on the information you provide; incomplete or inaccurate information can lead to an incorrect assessment or treatment.
- Technology failures (poor connection, dropped video, device issues) may delay or interrupt the visit and require rescheduling.
- Despite reasonable security and HIPAA-compliant safeguards, electronic transmissions can never be guaranteed 100% secure and remain subject to potential interception or breach.
- Delays in evaluation, diagnosis or treatment may occur due to deficiencies or failures of the equipment or the network.
5. Prescriptions and GLP-1 program
- A clinician will only prescribe medication, including GLP-1 receptor agonists (e.g., semaglutide, tirzepatide), when clinically indicated based on your history, vitals, labs and goals.
- Issuance of a prescription is never guaranteed by paying for or attending a visit.
- Controlled substances are not prescribed via this telehealth program.
- You agree to follow dosing instructions, attend follow-up visits, report side effects promptly via the secure messaging channel, and complete required labs.
- Your clinician may adjust the dose, switch medication, pause therapy or discontinue treatment at any time for safety or clinical reasons.
6. Privacy and protected health information
All telehealth visits and records are handled in accordance with HIPAA, our HIPAA Notice of Privacy Practices and our Privacy Policy. Your protected health information may be shared, only as needed to deliver your care, with the treating clinician, our partner pharmacy, contracted laboratories, your insurer (if applicable) and our payment processor. Visits are not recorded by GAB Medical Group unless you give specific written consent. You agree not to record the visit yourself without the clinician's prior consent.
7. Patient responsibilities
- Be in a private, well-lit and quiet location for the visit.
- Use a secure internet connection and a device with working camera, microphone and speaker.
- Have a current list of your medications, allergies and recent vitals (weight, blood pressure if available) ready.
- Notify the clinic immediately if you move to a different state or country, become pregnant, or experience new or worsening symptoms.
8. Fees, billing and refunds
Telehealth visits, program memberships and medications are billed as disclosed at checkout. Most visits are paid out-of-pocket and may not be covered by insurance. Refunds, when applicable, are governed by our Refund Policy.
9. Alternatives to telehealth
You may decline telehealth at any time and instead seek in-person care at a GAB Medical Group clinic or with another provider of your choice. Declining telehealth will not affect your right to future care.
10. Emergencies
Telehealth is NOT appropriate for emergencies. If you are experiencing chest pain, shortness of breath, severe allergic reaction, suicidal thoughts, signs of stroke, or any other life-threatening symptoms, call 911 immediately or go to the nearest emergency room. Do not use messaging or scheduling tools to report an emergency.
11. Withdrawal of consent
You may withdraw this consent at any time by notifying us in writing at info@gabflorida.com. Withdrawal will apply to future visits and will not affect care already provided.
12. Electronic consent and signature
By checking the consent box, scheduling a visit, or proceeding with the intake forms, you acknowledge that you have read and understood this document, that all of your questions have been answered to your satisfaction, and that you voluntarily give your informed consent to receive care via telehealth from GAB Medical Group as described above. Your electronic acknowledgement has the same legal effect as a handwritten signature.
